This form is only for distance education students taking courses at the University of Maine at Fort Kent and who do not attend any classes on campus.

If you are a student from away taking UMFK classes, you may fill out the form below. With the information you provide us we will determine if you need a UMFK Card and if so, provide you one in the mail. Please note that you can only receive a card if you have your immunization records and other forms up to date.

Getting a card means you will have access to the UMFK databases. These databases allow you to search through thousands of resources to retrieve articles and other materials available either in full text online, or in abstract and citation form so you may request to borrow the item.

Don't wait to apply. Once you apply it can take up to a week to get your card activated and mailed.
