Request for Approval of Credit
When taking courses from other institutions students are required to get approval from their advisor by submitting a request for an approval form. This form is completed each semester and includes all courses to be taken for the semester. By your signature on this form, you are giving the Registrar's Office at your home campus permission to request your official tanscript from the away school.
